Understanding the En Bloc Technique
Recently because of certain concerns about certain breast implants, a operation that seems to be trending is to remove both the implant and the capsule surrounding the implant at the same time. This may be called en bloc implant-capsulectomy. This actually is an operation that I did many times during the silicone adjunct study for implant and capsule removal. And to think about that, a good way to look at that is this baseball. If you consider the shell of the capsule as the covering of this baseball, and this is the implant within the capsule, what we would do is to remove the entire capsule, baseball, in its entirety without entering the capsule or the implant. And the reason why we did that was that with the older implants, the implant was filled with liquid silicone, and it would leak within this capsule, so we did not want to enter the capsule and have leaking silicone everywhere. Now with the concerns about certain implants, we can now again remove the entire capsule containing the implant en bloc, at the same time.
